New Patient Registration

If you live within the area covered by the practice please bring your medical card to reception or complete a registration form which is available from the receptionists. If your application is accepted you will be registered with the practice.

Temporary Residents

You are entitled to consult a doctor anywhere in the UK if you are away from home and in need of medical attention. You can do this by asking to see the local doctor as a temporary resident. We will be happy to see any of your friends or relatives if they need a doctor whilst staying in this area.

Non-English Speakers

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.

Pre-registration with the practice

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

Please ensure you are in the practice catchment area.  You can download and print off registration forms to fill in and bring down to the surgery.  You must hand these in to the Reception so they can be checked.  Postal registrations will not be accepted.

You are also required to bring proof of identity and address with you when you register.
